A Letter from Founder Dr. Sarah Merrick
Dear Patients & Friends,
Life is a journey, and the moments that matter most are precious. For the past several years, it has been my privilege to walk alongside you and your family through your own health journeys. I am grateful for the trust you have placed in me, and I am excited to share the next step in mine.
Beginning August 1st, I will officially open SummitMD, my own Direct Primary Care practice. The new office will be located just minutes from my current location. I look forward to continuing to provide the same experienced, personalized care you have come to expect—care that is guided by evidence-based medicine, informed by emerging research, and tailored to your unique needs and goals. I will continue seeing patients at MedLou through July 30th. I will begin seeing patients at SummitMD Monday August 10th. During the week of August 3-7th, the new office will be open, but we won’t be scheduling patients until the following week.
Opening SummitMD allows me to continue to practice medicine the way I have always envisioned: quality time for patients, accessibility, and a strong focus on prevention, wellness, and individualized care. My goal is simple—to provide exceptional medical care while building lasting relationships with the patients and families I serve.

To secure your enrollment and ensure uninterrupted care, simply fill out the Continued Care Agreement form below by July 24, 2026. After August 1, new patient pricing and enrollment fees will go into effect. Your enrollment at MedLou will end July 31. Questions regarding enrollment can be emailed to enrollment@summitmdhealth.com
A note on pricing:
Your monthly rate will increase by just $10/month/patient. This is the first adjustment since 2023 and remains one of the lowest rates in Direct Primary Care.

Patient Continued Care Agreement
For current patients of Dr. Sarah Merrick, the transition to SummitMD will be simple and seamless. To ensure uninterrupted care, you will need to fill out the Continued Care Agreement form below and return the paperwork we will send you
by July 24, 2026.
Please note - If you choose to continue care, we will send you additional details, including updated patient agreements, as soon as our medical records are transferred to SummitMD.

My goal is to give my existing patients the first opportunities to continue their care with me. Friends and Family will be able to sign up for the waitlist. As spots open up, we will contact prospective new patients.

My goal has always been to offer the highest level of care as a board certified internest to my patients, while keeping costs affordable.
That said, I have not had a price increase for my patients since 2023. In order to achieve the level of service to my patients, existing patient’s monthly cost will increase $10. Please see our pricing chart below to understand what your monthly costs will be as of August 1, 2026.
